Your answer to the discussion question may be typed or written legibly on a separate piece of paper. Please include your name, date and the title of this course on your discussion response.

Upon finishing the course, mail the answer sheet, discussion response, and evaluation form to:

Your test will be graded promptly. Upon scoring 80%, your Documentation of Completion will be mailed to you. Your answer sheet will be returned with corrections to facilitate further learning.

In order to issue your Documentation of Completion, we must have your completed evaluation for our records. The statement must be signed and dated to certify that you completed this course by your own efforts.
